NYSSB School Notification Form
The purpose of the NYSSB School Notification Form is to notify NYSED of a district's or school's intent to offer the NYSSB in the current school year.Ìý The deadline to submit this form is December 1st, in each year that the NYSSB is offered.Ìý
On this form, schools will provide basic information on the school, contact information,Ìý and numbers of student subgroups eligible to pursue the NYSSB.Ìý
